Take Charge! Live Well! is an exciting new health and wellness program brought to you by the state's labor management Joint Health Care Committee (JHCC).

Medical Providers and contact information

The first step toward a healthier lifestyle begins by logging onto the Take Charge! Live Well! website of your provider or by calling one of the toll free numbers listed below.

Initially, because each Take Charge! Live Well! provider has their own health management program branding and the program Take Charge! Live Well! has its own logo and look, there was some confusion concerning each health management program’s health assessment. To clarify:

• Aetna's health management program is called Simple Steps.

• APS Healthcare's program is called Healthy Together.

• United Healthcare's program is called MyUHC and uses the The University of Michigan health assessment.

To log into the APS Healthcare site to take the Health Assessment Log go to stateofohio.apshealthcare.com and use the 8-digit employee ID number when you are asked for "Member ID."

All technical issues associated with the roll out have been resolved. If members have experienced any difficulty logging into their designated provider's website, please try again.

Take Charge! Live Well! provides you with online access to valuable tools, information and services to assist you in making healthy lifestyle choices. 

 CONFIDENTIAL  Information submitted to your Take Charge! Live Well! provider is protected by the Health Insurance Portability and Accountability Act, a federal privacy law. Your individual health information is confidential and will not be shared.

 FREE  The program is available at no cost to State of Ohio employees and their spouses, ages 18 and older, enrolled in a State of Ohio Health Plan.
