Take Charge! Live Well! is a health and wellness program brought to you by the labor management Joint Health Care Committee.

The program is designed to improve the overall health of employees and their families… and to help manage the cost of healthcare benefits.

Developed with the typical state employee in mind, the program offers easy ways to get involved in the wellness program so you can start small or big, depending on your personal health goals.

Financial incentives—up to $200 when including spouse— participation are offered.

Information submitted to your Take Charge! Live Well! provider is protected by the Health Insurance Portability and Accountability Act, a federal privacy law. Your individual health information is confidential and will not be shared.

The program is available at no cost to State of Ohio employees and their spouses, ages 18 and older, enrolled in a State of Ohio Health Plan. 

More information will be mailed to members' home the first week of November.
