MORE SCREENING DATES

Get free health screening at your workplace this spring, summer

April 21, 2008 - The Take Charge! Live Well! Road Show is well under way, currently offering program information and free health screenings to many work locations across the state through June 25 (see list at right).

Earn $25 of your $100 Take Charge! Live Well! incentive by participating in the health screenings.

The three Take Charge! Live Well! providers will be available to conduct health screenings of:

  • blood pressure
  • blood glucose
  • total cholesterol
  • HDL and
  • Body Mass Index (BMI).

Employees enrolled in one of the state’s health plans plus their enrolled spouses – are welcome to participate. 

Your ticket in for the free health screening is your health insurance card from Aetna, OhioMed, Paramount, The Health Plan or UnitedHealthcare.

Check the BAS website at ohio.gov/employeebenefits for frequently updated information on dates and times the Road Show is coming to your area. 

Road Show events are open to employees who work in the facility, as well as employees from other state facilities. Check with your agency about attending an event at a neighboring state facility.

Also, members of the DAS Benefits Administration Services Team will be on hand to talk with state employees about the program and answer questions.

Note: In order to receive the free health screening, you must be enrolled in one of the state of Ohio’s health plans. Those who have Aetna, Medical Mutual, Paramount, The Health Plan, or UnitedHealthcare coverage through another employer are not eligible at this time.
