Retiree Membership

Notice Requests, Lists, and Labels

Online Meeting Notice Form - Submit your request online and receive a printable confirmation via the website and your home email.

Printer Friendly Meeting Notices Form (PDF) - Members can still use this printable form to request meeting notices and election notices in the Public Employee Quarterly. Only requests that comply with the OCSEA or AFSCME Constitutions and OCSEA policy will be honored. Submit request to Information Systems Section.

Lists, labels, notices Form (PDF) - Use this form to request meeting notices, election notices, membership lists or labels. Only requests that comply with the OCSEA or AFSCME Constitutions OCSEA policy will be honored. Submit request to Information Systems Section.

Financial Reporting - Download and print various forms used by subordinate bodies as well as a Treasurer's Resource Guide offering union accounting practices and resources.

Grievance Forms - Download and print the complete set or individual forms and checklists used throughout the grievance process.

Oath of Office (PDF) - Use this form to administer the oath of office to subordinate body officers, record officer information for newly elected or re-elected officers, and return form to OCSEA.

Delegate Change Information (PDF) - Chapter use this form to recognize delegates to an assembly or district council. Return completed form to the OCSEA.

Officers Change Information (PDF) - Following a subordinate body election, Chapters, Assemblies, and District Councils use this form to report newly elected or re-elected officers. Return completed form to the OCSEA.

Release Time Form (PDF) - This form is submitted to subordinate body treasurer when requesting paid release time for union business, per approved proper motion. Word Version
