FAQ: Members questions addressed on Take Charge! Live Well! incentive payments

May 20, 2008 - Take Charge! Live Well! incentive payments will be included in the May 23 paychecks of more than 26,000 state employees who participated (themselves or a spouse) in the program between July 1, 2007, and March 31, 2008.

Take Charge! Live Well!, introduced last year, is an exciting new health and wellness program brought to you by the state's labor-management Joint Health Care Committee (JHCC).

Here's a list of frequently asked questions:

I think the state made a mistake.  Why do I have $50 extra in my paycheck?

I did not “enroll” in Take Charge! Live Well!  Why do I see $25 in my paycheck?

I didn’t take the Health Assessment, but I got $50.  Why?

But I didn’t “apply” for the preventive care award.

I already received $100, the maximum incentive.  Why did I receive $25 more?

I had my annual physical, and I got $50 in my paycheck.  Why?

What types of preventive care are eligible for an incentive?


  1. I think the state made a mistake. Why I have $50 extra in my paycheck?
  1. You probably earned a preventive care award.
  1. I did not “enroll” in Take Charge! Live Well!  Why do I see $25 in my paycheck?
  1. Either you or your spouse (if spouse is enrolled in a state health plan) probably received a preventive care service from your doctor between July 1 and March 31.  There is no need to “enroll” to earn the preventive care incentive.
  1. I didn’t take the Health Assessment, but I got $50.  Why?
  1. Your incentive award is probably for two preventive care services.
  1. But I didn’t “apply” for the preventive care award.
  1. You don’t need to.  The Take Charge! Live Well! vendor electronically sorts through health claims, identifies those people who have received preventive care, and then notifies the state to pay your incentive award.
  1. I already received $100, the maximum incentive.  Why did I receive $25 more?
  1. You or your spouse probably earned a preventive care incentive.  A married employee whose spouse participates in Take Charge! Live Well! may earn up to $200.
  1. I had my annual physical, and I got $50 in my paycheck.  Why?
  1. You may receive up to $50 in preventive care incentives ($25 + $25).  Your annual physical may have included two separate services, such as an annual exam plus a cholesterol check.
  1. What types of preventive care are eligible for an incentive?
  1. The following preventive care services are eligible for incentive:
    1. Annual physical exam
    2. Cholesterol test
    3. Prenatal obstetrical care
    4. Mammogram
    5. Annual gynecological exam
    6. Pap
    7. PSA
    8. Colonoscopy
    9. Sigmoidoscopy
    10. Fecal occult blood test
