received after July 25, 2005 will not be considered by the convention.

Amendments
Proposal Form or - Return the completed forn to OCSEA, Attn: K. Brown, 390 Worthington
Road, Suite A, Westerville, OH 43082-8331 by 5:00 p.m. June 3, 2005.
Proposed amendments received after that will not be considered by the
convention.
